The Conversation Map® Program Engages Patients in Decision Making and Change

An overview
Each Conversation Map tool uses a series of images and metaphors to engage groups of 3 to 10 participants in conversations about a health care topic, such as diabetes.
This innovative teaching method is led by a trained facilitator. The role of the facilitator is to create a welcoming environment that uses the structured materials to guide group conversation and engage participants in a process of exploration and learning.
Identifying opportunities to change
The Conversation Map program provides participants with a process by which they can plan changes in their decision making and behavior. In some cases, the action plan is robust; in others, it provides a starting point for change. The Conversation Map program uses several components to support the learning process:
- A colorful 3x5-ft picture
that the group will focus on and work through during the session

- Conversation questions
that prompt participants to discuss various topics throughout the session.
- Discussion cards
that help engage participants in the learning process.
