Additional Resources to Enhance Patient Education

As part of the commitment to patient education that we share with insurers and health care providers, Journey for Control offers a wealth of patient resources. We’ve included some of them in this section so you can better understand how they can help meet the goal of facilitating behavioral change in your plan members.

These resources are designed not only to help patients with poorly controlled type 2 diabetes better manage their condition but also to provide support and reinforcement to those who are already taking the initiative to make healthy choices, control their blood sugar, and follow the treatment plan their doctor has specified.

One important, interactive learning resource is the US Diabetes Conversation Map® Program. Created by Healthy Interactions Inc. in collaboration with the American Diabetes Association (ADA), Conversation Map tools engage patients in a learning experience and provide them with a process by which they explore health facts through dialogue and draw their own conclusions. The Conversation Map Program experience is led by a diabetes educator who facilitates the learning process. The facilitator engages patients in interactive activities that identify facts and myths around various health care topics, such as proper nutrition.

Other valuable forms of information and support are readily available from the organizations listed on the last page of this section, through an assortment of online and printed diabetes education resources.
