The US Diabetes Conversation Map® Program Engages Patients in Decision Making and Change
Healthy Interactions created the US Diabetes Conversation Map® Program in collaboration with the American Diabetes Association and is sponsored by Merck & Co., Inc.
An Overview
Each Conversation Map is a series of images and metaphors—a tool that health care professionals can use to engage groups of 3 to 10 participants in conversations about a health care topic, such as diabetes.
This innovative teaching method is led by a trained facilitator. The role of the facilitator is to create a welcoming environment that uses the structured materials to guide group conversation and engage participants in a process of exploration and learning.
Identifying Opportunities to Change
The Conversation Map tool provides participants with a process by which they can plan changes in their decision making and behaviors. In some cases, the action plan is robust; in others, it provides a starting point for change. The Conversation Map tool uses several components to support the learning process:
- A colorful, 3-foot by 5-foot picture
that the group will focus on and navigate during the session.
- Conversation questions
that prompt participants to discuss a variety of topics at various points throughout the session.
- Discussion cards
that help engage participants in the learning process.
