Sponsored by Merck, and created by Healthy Interactions Inc. in collaboration with the American Diabetes Association , the US Diabetes Conversation Map® Program is designed to engage patients in a learning experience—one that allows them to explore health facts through dialogue and helps them draw their own conclusions.
Sessions are led by a diabetes educator who has attended a US Diabetes Conversation Map training session. In a casual group setting, the educator uses a series of images and metaphors to engage patients in conversations and activities around various health topics, including diabetes.
The Conversation Map tool uses several components to support the learning process:
- A colorful 3-foot by 5-foot picture that the group will focus on and navigate during the session
- Conversation questions that prompt participants to discuss a variety of topics at different points throughout the session
- Discussion cards that help engage participants in the learning process

Conversation Map sessions also include an action plan—an important part of the process that provides patients with a way to make changes in their choices and behaviors. For some patients, an action plan is robust; for others, it provides a starting point for change.
